I acknowledge that am the owner of the animal mentioned above. I ask the medical center to enter my animal and I acknowledge the following: First: I am obligated to come to the medical center to take my animal on the same day until six o’clock in the evening. Second: I understand that the cost is 4 Jordanian dinars for cats and 5 Jordanian dinars for dogs. Third: In the event of late receipt of the animal after six o’clock in the evening, the cost becomes 6 Jordanian dinars for cats and 7 Jordanian dinars for dogs, and the medical center has the right to charge the owner of the animal costs amounting to double the cost mentioned for each day increase for the prescribed period. Fourth: In the event that the animal is neglected for a period exceeding two days from the checkout date or the date of notification, the medical center has the right to present the animal for adoption or send it to one of the governmental or non governmental animal welfare centers and the owner of the above-mentioned animal loses his right to claim in the animal mentioned above Fifth: I have been informed of the possible consequences and risks of keeping the animal away from its home and the home of the original, and i understands that the medical center exerts its efforts to keep the sources of danger and diseases away from the animal, and I pledge not to hold the medical center or its representative any responsibility for any harm to it during the period of stay at the medical center Sixth: I authorize the medical center willingly to take the necessary medical, veterinary, therapeutic and surgical measures during this period and without necessarily returning to me. I also bear any material costs incurred by these measures implemented, whether in my presence or in my absence. Seventh: The medical center is not responsible for any damages related to civil or legal liability or compensation for moral or moral damages during the stay of the animal inside the medical center, and accordingly in the event that any harm, infection, disease, and/or everything that harms animal health is proven during the first month of stay, the medical center is exempt from any liability or compensation Eighth: I agree to grant permission to the medical center to photograph the animal via video or/and pictures and to use them in information materials and/or in educational materials as follows: audiovisual and readable media, printed publications, electronic publications or websites, and I authorize the medical center to use these pictures without any financial charge

أقر بأنني صاحب الحيوان المذكور في النموذج أطلب من المركز الطبي ادخال حيواني وأقر بالتالي اولا: ألتزم بالحضور الى المركز الطبي لأخذ حيواني في نفس اليوم لغاية الساعه السادسه مساءا ثانيا: أفهم أن التكلفة مقدارها 4 دينار اردني للقطط و5 دينار اردني للكلا ثالثا: في حال التأخر عن استلام الحيوان بعد الساعه السادسه مساءا تصبح التكلفة 6 دينار اردني للقطط و7 دينار اردني للكلاب وللمركز الطبي الحق في تحميل صاحب الحيوان تكاليف تبلغ ضعف التكلفة المذكوره لكل يوم زياده عن المده المقررة رابعا: في حال أهمال الحيوان مده تزيد عن يومان من تاريخ المغادرة او تاريخ التبليغ يحق للمركز الطبي عرض الحيوان للتبني او ارسالة الى احد مراكز رعاية الحيوان الحكومية او الغير حكومية ويفقد صاحب الحيوان حقة بالمطالبة في الحيوان خامسا: أنني أعلمت بالعواقب والمخاطر المحتملة جراء إبقاء الحيوان بعيدا عن بيتة ومنزلة الاصلي ,وافهم ان المركز الطبي يبذل جهده في إبقاء مصادر الخطر والأمراض بعيدة عن الحيوان ,وأتعهد بعدم تحميل المركز الطبي او من يمثلها اي مسؤولية عن اصابتة بأي مكروه خلال مدة الاقامة لدى المركز الطبي سادسا: أخول المركز الطبي بإرادتي اتخاذ الاجراءات الطبية والبيطرية العلاجية والجراحية اللازمة خلال هذه المدة وبدون الرجوع لي بالضرورة ,كما أتحمل أية تكاليف مادية تترتب على هذه الاجراءات المنفذه سواء في حضوري او غيابي وللمركز الطبي كامل الحق بتقرير الافضل لحيواني والمباشرة بهذه الاجراءات سابعا: ان المركز الطبي غير مسؤول عن أي أضرار تتعلق بالمسؤولية المدنية او القانونية او تعويض الأضرار الأدبية أو المعنوية خلال فترة الاقامة للحيوان داخل المركز الطبي ,وعلية في حال ثبوت أي ضرر و/أو عدوى و/أو مرض و/أو كل مايضر بالصحة الحيوانية خلال الشهر الأول من الأقامة فأن المركز الطبي معفي من أي مسؤولية او تعويض ثامنا: أوافق على منح الاذن للمركز الطبي بتصوير الحيوان عبر الفيديو أو/والصور واستخدامها في مواد إعلامية أو/و في مواد تعليمية كما يلي: الوسائل الإعلامية المرئية المسموعة والمقروءة المنشورات المطبوعة، أو المنشورات الإكترونية أو المواقع الإلكترونية وأصرح للمركز الطبي باستخدام هذه الصور دون أي مقابل مادي.
